Our Social Prescribing Service supports you to take control of your own health and well-being, make positive lifestyle changes and decide what you would like to do to feel healthier.
The Social Prescriber attached to our surgery is Kelly. She works with us one day a week and is here to listen to you and connect you with opportunities in the community that might help you feel better. This could include supporting you with things like managing your stress and loneliness, finding social groups, learning new skills, physical activity, or getting support with employment, benefits, housing and legal advice.
